Support Coordination

Support coordination is a simple process of evaluating, planning, finding, and coordinating services and support for people with disabilities. Our coordinators are always involved in assessing the needs of the individual to help them live the way they want. Discover our three levels of coordination below.

Level 1: Support Coordination
Elite Disability Care supports participants to understand their NDIS plan and begin using their funded supports with confidence. The team helps participants:

  • Understand their funding and how it can be applied
  • Connect with suitable local providers
  • Build confidence in making decisions and coordinating supports
  • Strengthen their capacity to manage their plan more independently

The focus is on empowering participants to take control of their supports while having reliable guidance available when needed.

Level 2: Support Coordination
Elite Disability Care provides a higher level of assistance for participants who require more active support to manage their NDIS plan. This service helps participants:

  • Understand their plan in depth and explore available options
  • Establish and maintain effective relationships with service providers
  • Navigate challenges and adjust supports as needs change
  • Build long-term capacity to coordinate their own supports
  • Monitor progress toward goals and ensure services are meeting expectations

This level is ideal for participants with more complex needs or multiple supports to coordinate.

Level 3: Specialist Support Coordination
For participants with highly complex needs or significant barriers, Elite Disability Care offers Specialist Support Coordination delivered by experienced practitioners. This service provides:

  • High-level coordination within complex service environments
  • Support to manage high-risk or crisis situations
  • Development and implementation of tailored, multi-disciplinary support plans
  • Collaboration with clinical, behavioural, and mainstream services
  • A focus on stability, safety, and sustainable long-term outcomes

Specialist Support Coordination ensures participants with the most complex circumstances receive expert guidance and strong advocacy.
